The choices of intraocular lenses (IOLs) available for implantation in cataract patients are multitudinous. Each option features significant benefits for visual performance and the comfort of the patients, but there are also some inherent limitations in each case. The traditional approach of monofocal IOL implantation is effective in providing adequate distance visual acuity, but is limited in its provision of near visual acuity.1 Monovision might also be considered, but the loss of depth perception through this approach has meant that not all patients are able to adapt to it2. Accommodative single-optic IOLs have provided varied results, often with moderate to no improvement in near visual acuity relative to control IOLs, and with lower contrast sensitivity (CS) outcomes3,4. Multifocal IOLs have been reported to provide better results with near and intermediate vision and to achieve a greater depth of focus and higher patient satisfaction5. Some multifocal IOLs, particularly early generation ones, have shown some noteworthy limitations, such as inferior CS results as compared with monofocal IOLs and increased higher order aberrations6.
